Sports PhysicalsCharles County Public Schools students who plan to try out for any school-sponsored interscholastic sports team are required to have a physical performed by a licensed physician.
A Charles County Public Schools physical form must be turned in to the school athletic director prior to the first day of practice. The first day of practice for fall sports is Aug. 15. No student is allowed to practice or try out for a team without a completed physical form signed and approved by a licensed physician. A physical is valid for one year. Parents and students are responsible for keeping physicals up-to-date.
Other forms needed before a student is permitted to try out are: authorization for participation in interscholastic athletics, emergency medical card, verification of insurance and a Statement of the Student-Parent Handbook. The school athletic directors or coaches provide these forms to all athletes.
The school system recommends that the student's primary care physician perform the required physical, as this physician knows the student and is familiar with the student's medical history. If a family physician is not available, the clinics listed below may be able to provide a sports physical. Call ahead to get specific costs and times.

Students interested in having their physical done at school should go to the school during the time frame listed. Students must have the parent portion of the physical form completed and signed along with the $30.00 (cash only) fee at the time of registration. Students will not be seen by the doctor without the required materials. Physical forms and permission forms may be obtained from the athletic director or home school front office. Physicals will be limited to the first 100 students and will be done on a first come first served basis.
Download and print the physical and permission forms here.
In addition to the physical, there are academic requirements for participation in sports. Students must achieve a minimum grade point average of 2.0 and may have no more than one failing grade per quarter in order to participate. All new ninth graders are eligible to participate the first quarter of ninth grade. Students are also ineligible if they are absent five or more days during a nine-week period.
Parents should call the school athletic directors with specific questions about sports participation.
